Start by filling in the tenant’s name, address, and county. Step 1 – The first page is the 10-day notice to quit. No other form of notice for possession or demand is necessary, however, the landlord will still need to fill out an Affidavit of Service after the notice was served which shows proof that the notice to quit was served. The notice to quit needs to be signed by the lessor or their agent and properly delivered to the tenant.
If the landlord elects to terminate the tenancy due to the breach, the landlord must give the tenant no less than 10 days’ notice to move out and deliver up possession.
